How to Request Service

Easily request service and track the status for a specific unit through the CTOS Customer Portal with just a simple click.

If you are renting a Custom Truck One Source unit, you can easily request service for that specific rental through your online portal. 

Before moving forward, make sure to create an account on the CTOS Customer Portal found on If you need help creating an account, please visit our helpful article: How do I register?

  1. First, head over to and be sure to log in.
  2. Once logged in, in the upper right-hand corner, hover over MY ACCOUNT and select Reports.
  3. Under SALES & RENTAL, select Equipment on Rent & Telematics - Zonar.
  4. Navigate to the Unit # column and select the equipment number link to view additional detailed information about the unit. Utilize the filter search bar to refine your results by entering information from any column (such as unit #, VIN, contract number, etc.).
  5. Click on the red button that says SERVICE REQUEST.
  6. Ensure that you complete all required fields in the form, including:
    • Urgency Level
    • Preferred Method of Communication
    • Phone Number
    • Service Type
    • Diagnostic Issue
    If you select Level 1 - Emergency, please contact the CIC at 833-281-7911 and/or 911 immediately rather than submitting a request. The portal does not support service requests at the Level 1 - Emergency urgency level. Instead, you will receive a warning message that must be acknowledged.

  7. Please upload photos and provide as much detail as possible to help the call center and service team address the issue as quickly as possible. When all required fields are complete, hit the SUBMIT CASE button.
